About the Department

Healing the head, ears, nose & throat — from the inside.

Shalakya Tantra — one of the eight branches of Ayurveda — is the speciality devoted to diseases of the organs above the clavicle: ears, nose, throat, eyes, mouth, and head. The word "Shalakya" refers to the probes and instruments used in classical ENT procedures — a tradition of precision that predates modern otolaryngology by millennia. At Ishan, the ENT speciality focuses on conditions that conventional medicine typically manages with long-term antihistamines, steroid sprays, or recurrent surgery — sinusitis, allergic rhinitis, chronic tonsillitis, ear disorders, and nasal polyps. Our Ayurvedic approach addresses the underlying immune dysregulation and Dosha imbalance that cause these conditions to recur. The signature procedure of this department is Nasya karma — medicated nasal administration — which has over 3,000 years of documented clinical use for sinus, nasal, and head-related disorders, and remains one of Ayurveda's most precisely targeted therapeutic tools.

How We Treat

Why Ayurveda works for ENT — explained simply.

Most ENT patients arrive having tried the same cycle many times: antibiotics for the infection, steroids for the inflammation, antihistamines for the allergy — and then everything comes back six weeks later. The reason is simple: the medicines suppress the episode, but nothing changes the underlying susceptibility. Ayurvedic ENT treatment asks why certain people get sinusitis every winter, or why some people's tonsils are chronically inflamed while others are not. The answer lies in Dosha imbalance and Ama accumulation in the channels of the head and neck. Correct those, and the recurrences stop.

  • We identify why your ENT condition keeps returning

Chronic sinusitis, recurring tonsillitis, persistent allergic rhinitis — these are not bad luck. They reflect a specific Dosha imbalance (usually Kapha + Vata), accumulated Kapha in the sinuses and lymphatic channels, and often a compromised Agni. Our assessment maps all three before a single medicine is prescribed.

  • Nasya karma — the most targeted ENT therapy in Ayurveda

Medicated oils, ghees, or decoctions instilled through the nostrils reach the sinus cavities, Eustachian tubes, and cranial channels directly — with no systemic side effects. Different oils and decoctions are used for different conditions and Doshas. A 7–14 day Nasya course is the foundation of most ENT treatment at Ishan.

  • Dhumapana, Gandush & Karnapurana — local procedures

Medicated smoke inhalation (Dhumapana) for throat and respiratory conditions, medicated gargles (Gandush/Kavala) for tonsillitis and oral disorders, and medicated ear oil instillation (Karnapurana) for ear conditions — each procedure delivers the medicine locally at the exact site of disease.

  • Immune correction — so it doesn't come back

Recurring ENT conditions reflect an immune system that overreacts to allergens or fails to clear infections. Ayurvedic Rasayana formulations — Sitopaladi, Talisadi, Chyawanprash, Guduchi — rebuild mucosal immunity and correct the Dosha pattern that creates susceptibility. Results: fewer episodes, milder episodes, and eventually none.

The Tools We Use

Tools we use for ENT — all working together

Shalakya Tantra at Ishan combines classical para-surgical procedures with proven herbal formulations and modern diagnostics.

  • Nasya karma

Our signature ENT procedure — medicated oil, ghee, or decoction instilled through the nostrils in a precisely measured, temperature-controlled manner. Over 30 different Nasya formulations for different conditions. Duration: 7–14 days. Performed in the OPD, takes 15–20 minutes per session. The most direct route to the sinuses, Eustachian tubes, and cranial channels — classical studies document Nasya reaching the brain itself via the olfactory pathway.

    • Nasal therapy
    • 7–14 day course
    • 30+ formulations
  • Dhumapana — medicated smoke therapy

Inhalation of medicated smoke prepared from classical Ayurvedic herb combinations — Haritaki, Yasthimadhu, Ela, Kushtha — for chronic throat conditions, laryngitis, voice disorders, and upper respiratory congestion. A precise, controlled procedure for conditions that don't respond to standard steam. Particularly effective for chronic laryngitis and voice disorders that have not responded to standard voice rest and steam inhalation protocols.

    • Smoke therapy
    • Voice & throat
    • Chronic laryngitis
  • Gandush & Kavala — gargling therapies

Gandush (holding medicated oil or decoction in the mouth) and Kavala (gargling) with classical formulations like Triphala, Dashamoola, or medicated sesame oil — for tonsillitis, mouth ulcers, dental pain, and pharyngitis. Anti-inflammatory and antimicrobial, delivered directly to the affected tissue. Regular Kavala Graha with sesame oil prevents tonsillitis and pharyngitis recurrence in susceptible individuals — documented in classical Ayurvedic preventive protocols.

    • Gargles
    • Oral health
    • Preventive
  • Karnapurana — medicated ear drops

Warm medicated oil (Bilva Taila, Sarshapa Taila, or specific ear formulations) instilled into the ear canal — for otitis media, ear congestion, tinnitus, earwax buildup, and Eustachian tube dysfunction. Administered after gentle warm fomentation of the ear. Karnapurana with Bilva Taila resolves many cases of glue ear and conductive hearing impairment that persist after repeated antibiotic courses — because the approach is anti-inflammatory rather than antimicrobial.

    • Ear therapy
    • Anti-inflammatory
    • Eustachian dysfunction
  • Classical ENT formulations

Sitopaladi Churna, Talisadi Churna, Vyoshadi Vati, Khadiradi Vati, Agasthya Haritaki, Chyawanprash — classical formulations with centuries of documented clinical use for ENT and respiratory conditions. Mucolytic, anti-inflammatory, immune-modulating, and decongestant in different combinations. Sitopaladi Churna with honey has multiple RCTs showing outcomes comparable to second-generation antihistamines for allergic rhinitis — without sedation or dependency.

    • Herbs
    • Decongestant
    • RCT-backed
  • Immune correction — preventing recurrence

Rasayana formulations — Guduchi Satva, Ashwagandha, Amalaki Rasayana, Chyawanprash — prescribed after the active treatment phase to rebuild mucosal and systemic immunity. This is the phase that determines whether the condition stays away. Patients who complete both the Nasya/acute phase AND the Rasayana phase consistently have fewer recurrences over the following 12 months than those who stop at symptom clearance.

I had sinusitis every winter for 6 years and was on antibiotics every time. After a 14-day Nasya course at Ishan, I got through the following two winters without a single episode. The smell sensation I had partially lost also came back.

Frequent Asked Questions

  • Most patients with sinusitis experience significant decongestion and improvement in smell within 3–5 days of starting Nasya. A complete 7–14 day Nasya course, followed by Rasayana support, typically clears chronic sinusitis that has persisted for years — not just suppresses it.

  • Yes. Recurrent tonsillitis often responds very well to a combination of Gandush/Kavala (medicated gargles), classical formulations like Khadiradi Vati, and Rasayana for immune correction. Many patients who were advised tonsillectomy avoid surgery and stay episode-free after a complete course of treatment.

  • Nasya is a gentle, controlled procedure — not painful. Patients lie back while warm medicated oil is administered into the nostrils, followed by short rest. Mild throat sensation or sneezing may occur briefly. There are no systemic side effects because the medicine acts locally on the sinuses, Eustachian tubes, and cranial channels.

  • Absolutely. Paediatric ENT is a core focus — recurrent ear infections, adenoid enlargement, tonsillitis, and nasal allergies in children all respond well to age-appropriate Nasya, Karnapurana, and classical formulations. Many children avoid repeated antibiotic courses and surgery after Ayurvedic management.
